Reeves County Emergency Services District Nos. 1 & 2 may charge fees for public information requests as allowed under Texas Government Code Chapter 552 and the Office of the Attorney General’s Rule 70.3. These fees reflect the actual cost of compiling and providing requested information. A detailed schedule of charges and additional guidance is available by clicking here.

1 – Complete the Public Information Request Form.  If the requested record contains Protected Health Information (PHI), please also complete the HIPAA Authorization Form.

2 – Submit the form(s) to Reeves County Emergency Services District Nos. 1 & 2 by one of the methods below (email is preferred)
